Charles County Local Care Team

The Local Care Team is a team of professionals from child serving agencies in Charles County that gather together at one table to identify needs, make connections, advise and assist families with children who have intensive mental, emotional and behavioral needs. 

The members of Charles Counties team include:

  • Charles County Advocacy Council for Children, Youth and Families (CCACCYF)
  • Department of Juvenile Services
  • Department of Social Services
  • Developmental Disabilities Administration
  • Charles County Public Schools
  • Behavioral Health Authority
  • Division of Rehabilitation Services
  • Charles County Department of Health
  • MD Coalition of Families

Under Maryland Code (Human Services Article 8-407), a local care team shall:

  1. Be a forum for:
    • Families of children with intensive needs to receive assistance with the identification of individual needs and the potential resources to meet those identified needs; and
    • Interagency discussions and problem solving for individual child and family needs and systemic needs;
  2. Refer children and Families to:
    • Care management entities when appropriate; and,
    • Available local and community resources;
  3. Provide training and technical assistance to local agency and community partners;

4. Identify and share resource development needs and communicate with the care management entity, local behavioral health authority, provider networks, and other local care teams in surrounding jurisdictions
