Description of Services:

We work to assist Senior Citizens (persons 60 years of age and older) and Young Disabled Persons (Deemed Disabled by Social Security Administration) with Enrollment into Medicare, Medicare Advantage, Medicare Part D and Medicare Savings Programs. Conduct Level 1 Screens (the intake screen to access long-term care services and the Medicaid Waiver Program). Provide Senior Citizens and Young disabled persons with Information, Assistance, and referral to programs that may help them navigate emergent needs.
